Insurance

Blackburn Physical Therapy is a preferred provider for Blue Cross Blue Shield (BCBS) of Vermont only. For all other insurances, visits with Blackburn Physical Therapy are out-of-network and payment via check or charge is expected at the time of the visit. Documentation for bill submission for out-of-network benefits is available upon request. Blackburn Physical Therapy is not credentialed by Medicare. Only services not covered by Medicare can be provided by this office and patients cannot submit bills to Medicare for reimbursement for these services.

$5 discount for paying with cash or check at time of service.
